All-on-4 Implant System: Detailed Patient Information

We would like to provide you with detailed information about the All-on-4 (All on Four) system, a revolutionary solution for the treatment of missing teeth. This system offers a modern, fast, and comfortable treatment alternative, especially for patients with a lack of jawbone and those who are completely edentulous (missing all teeth).

1. What is the All-on-4 System?

The All-on-4 system, as its name suggests, is based on the principle of fixing an entire arch prosthesis onto only four dental implants placed in a single jaw (upper or lower). This method eliminates the need for separate implants for every tooth, which is common in traditional implant treatments.

The Core Philosophy:

  • Reduced Implant Count: A full-arch prosthesis, which might traditionally require 6 to 8 implants, is supported by only four implants.
  • Angled Placement: The two implants in the posterior (back) region are placed angulated (at an angle), which is different from the standard. This strategic placement ensures the implants anchor into the most solid and dense parts of the jawbone (the anterior region, areas close to the chin, and near the sub-nasal region in the upper jaw).
  • Avoiding Sinuses and Nerves: The angled placement allows us to bypass anatomical structures like the sinus cavities in the upper jaw (which often require surgical intervention) and sensitive nerve channels in the lower jaw. This frequently eliminates or minimizes the need for additional bone surgery (like sinus lifting or bone grafting).
  • Immediate Loading Capability: When appropriate surgical conditions are met, a temporary fixed prosthesis can be attached on the same day or shortly after the implants are placed. This means the patient completes the treatment process without remaining toothless.

2. When is All-on-4 Applied?

This system is an ideal solution for the following conditions:

  • Complete Edentulism: Patients who have lost all their teeth in either the upper or lower jaw.
  • Bone Resorption Due to Long-Term Tooth Loss: Cases where the jawbone volume, particularly in the posterior areas, lacks sufficient height and thickness for implant placement due to years of tooth loss or other factors.
  • A Desire to Avoid Removable Dentures: Patients who are unhappy with the movement, difficulty in use, and reduced sense of taste caused by traditional removable prostheses (dentures) and are seeking a fixed solution.
  • Seeking a Quick Solution: Patients who wish to obtain fixed teeth quickly with a single surgical procedure and minimal additional procedures.

In summary, the All-on-4 system aims to achieve a full-arch fixed prosthesis with 4 strategic implants in cases of complete edentulism, especially when bone deficiency prevents the use of traditional methods or requires long and complicated bone augmentation treatments.

3. The Importance of Price Difference and Prosthesis Quality: Undeniable Facts

The All-on-4 system is not a simple set of screws but a complex feat of engineering. The success and longevity of the treatment depend directly on the quality of the implants used, as well as the quality of the prosthesis fabrication and its reinforcement system. Unfortunately, the greatest danger behind the extremely low prices offered in the market lies precisely here.

A. The Load on the Prosthesis and the Risk of Fracture

The All-on-4 prosthesis must withstand the chewing forces of an entire jaw while being supported by only four small anchor points (the implants). This means it operates under a much higher pressure and load compared to traditional implant systems.

Therefore, the structure that must be included in your prosthesis is:

  • Titanium Bar (Metal Reinforcement System): It is an absolute necessity to place a strong and flexible Titanium (or Chrome-Cobalt) Bar that supports the entire prosthesis inside the final All-on-4 prosthesis. This metal framework prevents the prosthesis from bending, cracking, or most importantly, fracturing during chewing. This bar ensures the prosthesis’s durability and stability.

B. The Hidden Danger of Cheap Applications

To reduce costs and offer attractive prices, some clinics make a vital mistake in the market:

  1. Exclusion of the Titanium Bar: This critical titanium bar system is not placed inside the prosthesis.
  2. Use of Temporary Material: The permanent prosthesis is made from materials with low durability, often the very same material used for temporary prostheses (which should only be used during the first few months of healing), because it is cheap and weak. This allows the prosthesis to be produced at at least three or four times cheaper than its normal cost.

Consequences and Patient Distress:

  • High Fracture Risk: These cheap prostheses, which lack a titanium bar or are made from temporary materials, cannot withstand high chewing pressure and soon fracture or crack (usually within the first year).
  • Denial of Responsibility: Unfortunately, when faced with this situation, the patient is often told, “You must have clenched your teeth too hard, put too much load on them, we are sorry, but we cannot do anything about it,” effectively denying responsibility. The patient not only loses the money they paid but is also forced to have a new, correct, and much more expensive prosthesis made.

C. Correct Application and Visual Evidence

It is our priority to ensure our valued patients are not misled and receive the correct treatment. We visually demonstrate the difference between the system applied in our clinic, which adheres to international standards and is the correct application, and the wrong/faulty system sold cheaply in the market that excludes the titanium bar.

  • (At this stage, please show the patient the visuals of the correct and incorrect prostheses and the titanium bar inside them)

In the correct system, the metal framework that strengthens the prosthesis and shines through is clearly visible, while in the cheap system, this critical reinforcement is completely missing, and the prosthesis consists only of weak acrylic (plastic) material.

Remember, implant treatment is an investment that will remain in your mouth for many years. The reason for the price difference is the compromise in quality and materials. Please focus not only on the price but also on the long-term durability of the treatment, the engineering of the prosthesis used, and the transparency of your doctor when making your decision.

The correct All-on-4 provides a lasting smile and comfort. The cheap one often brings permanent problems.
